Patient requires a 24/7 care plan to ensure a safe discharge transition since it was determined patient unable to return to their own home alone or needs temporary placement.

MyBridgeSuite — select a location if home is not an option

Hospital submits referral
Face sheet, H&P, consults, provider notes and summaries is sent through MyBridgeLink.

BridgeCare Health Network mobilizes the care plan
Full coordination from arrival through ongoing support.
Care agency arrives to begin 24/7 support upon patient arrival
Coordinates and facilitates next-step services for Skilled Home Care or Hospice
Transportation is coordinated as needed

Digital forms & consent
BridgeCare Health Network connects to obtain e-signature for Patient Registration, ROI/HIPAA, MyBridgeCare and MyBridgeSuite agreements.
