BridgeCare Support Suites Workflow — Owner / Property Partner

Referral Notification & Review
  • Receive real-time alert (email / text)
  • Log into Dovaxis dashboard
  • Review priority referral — Rapid Ready
  • Target: initial review within 15 minutes
  • Review discharge timing, patient needs impacting environment (mobility / fall risk, dementia / behaviors, bariatric), and equipment needs (hospital bed, oxygen, commode, etc.)

Suite Capability Decision
  • Can the environment safely support this patient?
  • Is a room available and ready within required timeframe?
  • Respond within 2 hours — Accept or Decline (with reason)

If ACCEPTED → Suite Preparation
  • Confirm room assignment
  • Prepare environment: hospital bed + linens, safety equipment (commode, walker, call system), clean and staged for arrival
  • Complete Welcome / Setup Checklist

Coordinate Care Overlay
  • Work with Central Intake to confirm:
  • PDHC staffing (if required)
  • Skilled home care or hospice agency
  • Any special equipment delivery

Confirm Back to Central Intake
  • Update Dovaxis — Status: ACCEPTED, room ready time, any limitations
  • Central Intake will finalize placement and notify hospital

Pre-Discharge Readiness
  • Suite fully prepared prior to arrival
  • Access instructions ready
  • Staff aware of arrival timing

Day of Arrival
  • Patient arrives via arranged transport
  • Safety check and orientation to space
  • Ensure immediate comfort and access to essentials

Ongoing Coordination
  • Maintain communication with PDHC / hospice agency and BridgeCare Central Intake
  • Report any concerns or changes promptly
